Here are some documents you’ll need when coming for your appointment at CFCH

  • NRIC/EP/Passport
  • Corporate insurance member card/e-card (if applicable)
  • Referral letter (if available)
  • Relevant medical reports/results (if available)

  • For patients from certain countries, a valid visa may be required to grant entry into Singapore.
  • For more information, please visit https://www.ica.gov.sg/visitor/visitor_entryvisa.
  • Immigration & Checkpoint Authority (ICA) website to check on the requirements.
  • Should you be required to remain in Singapore beyond the period allowed with your short-term visit pass, speak to our staff and we will assist you in extending your stay. 
  • Do note that you will need to present the following documents to extend your stay in Singapore:
    • Passport
    • Doctor’s letter
    • Application form for a visit pass (Form)

Insurance plans may be necessary in meeting the cost of oncology treatments and procedures.

With a Letter of Guarantee (LoG), direct billing arrangements can be made with your insurer for your convenience.
