Here are some documents you’ll need when coming for your appointment at CFCH

- For patients from certain countries, a valid visa may be required to grant entry into Singapore.

- Immigration & Checkpoint Authority (ICA) website to check on the requirements.
- Should you be required to remain in Singapore beyond the period allowed with your short-term visit pass, speak to our staff and we will assist you in extending your stay.
- Do note that you will need to present the following documents to extend your stay in Singapore:
- Passport
- Doctor’s letter
- Application form for a visit pass (Form)
Application for extension of stay on medical grounds (Form)
Insurance plans may be necessary in meeting the cost of oncology treatments and procedures.

With a Letter of Guarantee (LoG), direct billing arrangements can be made with your insurer for your convenience.
